I agree with current med Dx & Tx. Great text, $60, every medicine resident I've met has and loves this text. It may not have much pathophys, but I thought it's focus and brevity was straight to the point and told me what I needed to know about diseases X, Y and Z. Plus, it's easier to tote around than huge harrison's. My medicine rotation focussed little on pathophys (just the basics) and much more on treatment. Plus I have Robbin's, so if I really needed pathophys info, I could go there.
My school and hospital have an online subscription to Harrison's, UpToDate and MD Consult. Most places also have these subscriptions (or combinations thereof) so I would find out what your school has before buying. Get a text that compliments your online resources (chances are, it'll be easier to look something up on a computer and print it out whenever you are in the hospital).
