So I am stuck. Today, I was trying to count 5 recommenders off the top of my head and was wondering if the following seem to make any sense:
1. math/phys prof (intro course)
2. orgo chem prof (went to office hours but I doubt she remembers me after 3 years)
3. research prof (knows me well)
4. research prof (knows me ok? but I don't know prof that well...somehow seems strict)
5. humanities professor (took class soph year but really great person and I am pretty sure remembers me...just need to catch up)
6. program director (something I was part of that is academic)

*What I'm concerned about is how am I coming across as an applicant if my course professors are people that I have taken intro classes with? The ones that I took upper level sciences with were not as good so I think it would be a waste of time.
*Note: I work currently but I don't want my supervisor to get involved in the next phases of my life and I feel like I just wasted 2 years of my life being at a job that doesn't return anything of value to me on paper. I don't mind people contacting supervisor but my job really wasn't so fun for me and quite the opposite.
 
I don't think it matters much at all if the professor taught you an intro class vs upper level. Just try to choose people who know you, who you have a good relationship with, and who you know will have positive things to say about you and your work. (Don't choose the prof whose class you slept through all semester, etc.) If at all possible, try to stick with people who you know will remember you. But if you absolutely have to choose someone from a while back, send them a copy of your CV or a brief description of what you've been up to since you last took a class with them.

I see that you don't want to ask your work supervisor. But what about someone from one of your EC's? You must have some clinical experience, right? Anyone you can ask from that or from volunteering?
