Granted I'm just a humble 2nd year, but I imagine the most common red flags are things like:

Step failures
Course failures
Non-emergency related leave of absence (not as sure about this one)
Legal issues
Noted professionalism issues on a dean's letter

Being an IMG is a red flag in it's own right.

I think I generally agree with this.

But that said, there are various types of IMGs. My guess is IMGs from developing nations or Americans who got rejected from US allopathic and DO med schools and applied overseas = red flag. I believe both these categories account for the most IMGs, but not 100% sure, I'd have to look at the NRMP match data.

However, I've met IMGs from UK/Commonwealth nations for example who have pretty solid resumes (e.g. MD/PhD). Here is one example of an IMG who got into an MGH neurology residency. Here is another IMG who did most of his surgical training in Australia, came to the US for fellowship, and now works at MGH.

USMG or IMG with Professionalism issues (big ones) > IMG status (with imperfect record, e.g. Step failure; OR without completed Step I, II CK, II CS, and III scores in by time of application) > USMG with Step failures > USMG with single clerkship failure (in specialty you are not applying for) > IMG status (with perfect record and above-average Step scores) > USMG with medicre grades in clerkship of specialty applying to > USMG with m2 course failures (successful make-up) > USMG with m1 course failures (successful make-up) > non-emergency leave > emergency leave

You seem to say this authoritatively but what's your source, just gut feeling?

Only start doing that once you become a USC med school dean, but be a little more cautious.
