Redosing meds

What do you guys generally do in terms of redosing medications after major blood loss and transfusion? For example when you lose 1 blood volume and tranfuse it. Do you redose your antibiotics? Paralytics? Sedatives?

What if you give a lot of albumin? (such as in liver patients). Do you then give more of medication that are highly protein bound?

Our institutional policy is to redone antibiotics after over 1L of blood loss. For everything else, you can monitor effectiveness other ways (twitch monitor, end tidal, etc...).
