Increasingly over the years I have selectively referred to Physician only subspecialist groups - when I have the choice. And increasingly when I don't have a choice, indicating on referral, or verbally to patients request this specific Physician.

How often are y'all going the extra step to get your patient to a Physician for there other medical needs?

I actually spend more time then I would like discussing with my patients when I think their current FPNP or PA might not be suitable for the complexity of their child's needs. It helps that our peds department is accepting patients and is not an OSH referral. I do a similar routine when referring difficult therapy cases to PhD psychologists versus standard master levels therapist. Many people who do not understand the intricacies of medicine do seem to understand the benefit of a specialist who "only does this one thing, all day, every day".

As an aside, I recently had a father bring his second son to see me who was being managed for straight forward, stimulant responsive, ADHD by a NP noting "I wouldn't bring a car wreck into a paint store" as his stated chief complaint when asked why his son was here. Some people really do get the difference.
