Sorry your post is difficult to read due to grammar issues. Do you mean "doing a MD-PhD in the US, after finishing PhD in India"?

in short, there can be many barrier in such way. Any MD program (including MD/PhD program) would be extremely difficult for people without green card AND US formal education (usually a US bachelor degree). A more practical way might be to come for a post-doc position. Try to publish as many paper as possible to get green card through Eb1a. With a green card and some good US research experience, you should have a good chance to apply for a MD program. No need for PhD as you already did PhD in India and post-doc in US
