I am a foreign medical student. How long do they take to process an application and issue scheduling permit? If I register for step 1 in the first week of January , will I be able to get my test date in march?

It depends on the efficiency of your home school's front office. If they can return the ECFMG verification papers quickly to Philadelphia, then you will be issued a permit within a week or 2 of that. For me, it took about 3-4 weeks.
