So I just payed $1600 for step 2 CS and CK and saw this bolded clause

To avoid a delay in processing, your Certification of ID and Authorization Form must be complete, signed and certified as instructed

1)Where can I find this form at.
2)Do I even need to fill it out again since I did it for step 1
