I'm trying to get this done myself. You have to go here: http://oag.ca.gov/cures-pdmp

Click on PMDP registration on the right side of the page and then Pharmacist. You fill out the registration form, print it out and have it notarized. You will need a photocopy of a government issued ID, a copy of your pharmacist license and I think a copy of your store's DEA registration. You need an email address that is only accessible to you which you will need to verify within 72 hours.

I tried to register a couple months ago and abandoned the process before I sent in my forms. This was a big mistake because the site wouldn't let me try again and the help desk is very difficult to reach on the phone. I advise completing the process right away once you start it. I'm not sure how you access the database at work because I haven't got that far.

Thanks Diastole

Can anyone confirm this is the procedure?

I started the registration process by filling out the form online. I haven't received my paper pharmacist license..so I can't mail in the notarized copies yet.

Yes . This is the procedure. When you filling out the registration form make sure print it immediately otherwise you can not retrieve it later. Make sure to notarized everything ( registration form, copy of pharmacist license , copy of ID, copy of pharmacy DEA registration).I just got my access code recently via email about 1 month after submitting the document..

Maybe a stupid question... After you print out the form, it asks you to NOT sign and date until authorized to do so; am I supposed to wait for another email after I do my email conformation? When am I supposed to send in those documents? I'm so confused...

You have to sign it in notary office (at the time they ask you to do so)when you notarizing all document.

Thank you, so all I can do now is just wait for the board to send me another email allowing me to take all my documents to a notary office?

You don't have to wait for any email. Just take all documents including registration form to notary office and then send it to cures for approval.

I also got locked out of cures because i didnt register fast enough. The CURES staff is non existent. I gave up trying to register after several months of unanswered emails and a few dozen phone calls trying to get a hold of anybody. I literally tried every 15 minutes one day to call them... all i got was their stupid recorded message. Becareful to do the paperwork right the first time or you will never get cures access.

Sorry to bump this thread, but I am currently trying to register as well. We need to send a copy of the pharmacy DEA license? I assumed that part was for MDs and anyone else who carried their own DEA license.

haha, vardenafil...I have your hookup..pm me

I took the DEA license off the wall of the pharmacy, made a copy, and brought it to the notary. I think they do that to make sure that you are actually employed in a pharmacy and not just looking to see if Paris Hilton gets any controlled medications for fun.
