Repeat Delete After Transfer - Please Help

So I have a quick question for the vets in applying. In my transcript section I am having difficulty figuring out what to do with a course. I took Calculus II three times. Twice at McGill and once at CSU, I recieved a D and F and an A+ in that order. The course numbers are obviously different being from different schools, but both are run of the mill Calc II. Should I enter it as repeat delete (how do I do that) or just leave it as a thorn in my GPAs side (I graduated with a 3.912 in Biochem, but my first two years bring it all down to 3.2 cum.).
 
You enter it with a code as "replaced grade". All three calc 2 courses should have the same code so they track which grades to replace. (at least that's how it used to work... )

Each course the has a special selection will have a number (1,2,3 etc). Every time you repeat a course, it will have a letter code, and the numerical code referencing the course.

Calc II R1 Repeat 1
Calc II R1 Repeat 1
Calc II F1 Final 1.

It is something like that. I may not have the codes right

If they did I could easily replace them, but because I trasfered universities the first two are MATH 140 Calculus II and the last is MATH 255 Calculus II. Even though they are similair courses at different institutions, should I still replace them, they do not have the same codes.

You assign the codes, it is not based on the course number given at the college
