Hey all- kind of a funny thought/question, but here it goes- if trying to change specialties and rather than continuing in a current specialty- but to avoid "being out of medicine for nearly 20 months" would it look ridiculous to do another internship- say a transitional internship? has anyone seen this done before?

Yes.

Honestly, I don't think being out of medicine for one year will be a huge disadvantage. You're going to be learning new skills in your specialty anyways. No need to bring on the pain earlier than it needs to be.
