Replacing a grade

I searched for this but couldn't find it.

My understanding is DO schools let you replace your grade. What I want to know is how much of a match does the class have to be?

I got a C in 1st quarter calculus and anatomy/physiology, if I take them at a different school with different class numers, etc can I still replace them?

How do they check the content?
