Reporting ANC for clozapine in REMS

Ultimately does that usually fall more on the prescriber or pharmacist? While in my first two years of residency, i always had to because the pharmacist at the facility did not want to mess with it. However, my clinic years of residency, the pharmacies typically reported the ANC, and all I would do is just check to make sure it was ok. I never had to report ANC 3rd/4th year. However, now it seems the pharmacies in my current location have no desire to report ANC. Just curious what others experience has been.
 
In all settings that I worked in as a resident (inpatient and outpatient) as well as in my current inpatient position, we inputted the ANC into REMS. How would a pharmacy even have the ANC, assuming you aren't working in a large healthcare system?

if i remember correctly, the lab results would be faxed to the pharmacy
 
Our clinic typically reports to Rems for the pharmacist to check. Ideally it’s done by the RN or MA in our clinic but sometimes I’ve had to do it myself. I’ve worked in 3 systems in 2 states and never had a pharmacist do it.
 
Inpatient pharmacy handled it at our units in residency. I didn't end up managing clozapine on any outpatients through the big academic hospital. I did have a couple at the CMHC which had its own pharmacy (who also took care of it). No current clozapine patients but we have some nurses who handle putting it into REMS. I think we also gave our clinical pharmacists proxy access but I don't think they actually plan on entering any info most of the time.
 
Anyone else had an issue with REMS saying you are registered to give medication to a given patient, but then the pharmacy is getting an error message that you aren't?

is that just since the new system started or been happening for you?

also, can we no longer report anc on website, it looks like now we have to fax that form??
 
Pharmacy has always done it at our inpatient units. A year or two ago our outpatient clinic hired a full-time pharmacist who handles REMS for us.
