Reporting Rx Errors

Hi,
I was just wondering if anyone could tell me the website/organization that is used to report mis-filling errors in a pharmacy... I'm a pre-pharmacy student and I know that the "what would you do if you gave a patient the wrong rx" comes up in interviews a lot...

I would only go to ISMP if it were an adverse reaction.

If it were a misfill, I'd follow my internal corporate or employer instructions on incident reports first. If it were truly an adverse reaction, then, after receiving approval, I'd report it to ISMP.

But...that's just me. It gives the lawyers a heads up first.
