I would talk to the cardiology attending and see if they can write you a letter. You should have cards attendings write letters for you, not GIM attendings (other than your PD).

I used one research letter and three clinical letters. It was fine. All of them were pretty good and got me interviews.
 
My research letter would probably be lukewarm at best then....

Should I just stick with the three clinicals? It is necessary to get a research letter ?

It's only really "necessary" to get a research LOR when you have a PhD or you did a research fellowship that meant you took time off during med school or residency. If it's just a research mentor who doesn't know you clinically and it's not a huge part of your CV, no need for it. It can't really hurt but it probably won't help either.
