I wouldn't go as far as calling it a "must" but I would say "strongly recommended". Especially because many of the top programs are bigger academic institutions, PDs appreciate interest in research.
And really, there's a HUGE variety of research projects - ranging from big trials to case reports, it shouldn't be too hard to find one - many residents would love to have students help them write something up. 😀

How much? I was involved in a pedometer/balance study with two elderly populations for a year the year before my first year in MS. At this point (MS2 survivng systems and studying for boards) I dont have much time to spare for research (let alone catching my breath). I work at a free clinic doing diabetes counselling for the local community. Would a PD view that as important or should I ween myself from that program to attain more research experience? Thanks for your time in advance. andy

well if you need a quantification of research, anything less than an abstract/poster presentation is irrelevant.
your work at the free clinic is a different entity which would certainly be viewed highly but is not comparable to research - they are apples and oranges.
