If I got published as second author or lower does that publication even matter when applying for residencies?
 
Yes. They will go on ERAS. People will ask about them. It's not first author, but it's far, far, far superior to nothing.

OK so when I'm looking at the NRMP stats where derm or whatever competitive specialties that matched had an avg of 5-7 publications that's not just first or 2nd authors but everything is counted there? Because having an AVERAGE of that many solid publications seem crazy?!

That's not just publications; that includes posters and abstracts.

And, no, that's not just first or second authors.

One summer of research can net you a publication, two posters, and two abstracts. It's not that hard to build a nice little research resume if you make it a priority. Not every neurosurgical applicant has a half-dozen first authorship Nature/NEJM papers.
 
If you submit an abstract and it gets accepted for a presentation it only counts as one research experience, not two.

How about abstracts for two different presentations? Or can an abstract not be re-submitted once it has been published for another presentation/conference?

It depends, generally, though, it's ok to submit the same abstract to say, a school poster session, the state mtg, and a national meeting, but listing the same abstract 3 times on your cv isn't going to necc get you much mileage. Most people update/revise a bit and then they can make it a slightly different abstract.
