I got this from my advisor. It is the results of a survery sent to all program directors in pediatric residency program throughout the country. 60% of all programs responded. I have no idea which programs applied so I cannot comment on whether it pertains to a certain type of program (eg, competitive vs noncompetitive, academic vs community, etc.). The scale is 1 through 5, with 5 being the most important.

Criteria Used By Pediatric Residency Directors to Screen for Interview
Clinical Performance - 4.67
Dean's letter- 4.14
Letters of rec- 3.92
Number of honors- 3.80
Class rank- 3.74
USMLE scores- 3.48
Preclinical performance- 3.40
Medical school attended- 3.21

Criteria Used by Peds Residency Directors in Ranking Applicants
Interview- 4.58
Clinical Performance- 4.54
Dean's Letter- 4.06
Number of honors- 3.70
USMLE scores- 3.59
Class rank- 3.58
Preclinical performance- 3.57
Medical school attended- 3.27
Applicant interest in the program- 2.89

What surprises me the most is the relatively high weight placed on number of "honors," which varies widely among schools. Some schools are only pass/fail, others give no more than 15% of the class honors, others give >50% honors. USMLE scores are not among the most important factors either.

Where do letters of recomendation fall into this system?

This criteria is better to use to compare different programs to one another, like peds to ortho, for example. It's kind of nebulous all by itself, I think.
