I have heard from talking to people that some fill out forms with ratings for each aspect of a residency program in order to help with evaluating the program and ranking the program. Anybody else heard of this or have any examples. Thanks.
 
You mean a form that you can use on your interview trail? APGO has a nice one--I'll attach it here.
