I'm a little confused about the sequence of residency programs. Do you do a transitional year for PGY1, and then a neurosurgery for the next six years, or is there general surgery involved somewhere there as well? Thanks.

your pgy1 year is a surgery internship. TY is not allowed.

the next 6 (or 5 at a select few programs) are all ns.

just to add, 1 or 2 of those years in those NS training years are kept aside for research, so it's not always that youre operating all those six years.

some people get an "infolded fellowship" during that time. other places are so busy that you come in and do cases (not as many as usual) during your research year(s).

but all in all, 1 general surgery year (actually something like 9 months gen surgery and 3 months neurology) and 5-6yrs nsurg

Its not a general surgery residency. The first year of your NS residency is devoted to general surgery.

Most NSGY programs are 7 years long, and they include research. After residency, there are fellowships, but most neurosurgeons do not do one. There are fellowships in: complex spine, skull base, endovascular, functional and stereotactic, pediatric, tumor, cerebrovascular, maybe some others.
