For MD, not many that take a significant number of OOS. West Virginia if you are in the region. The Maine track for Tufts potentially. Vermont too as their class is majority OOS.

For DO, there are many you could look into:

WVSOM
ACOM
Campbell
WCU
KYCOM
VCOM
PCOM South Georgia (likely the Georgia campus too)
UNECOM
 
EVMS




Michigan State has some rural campuses I believe for both MD and DO and takes enough OOS students that it may be worth applying. Central Michigan is rural and about 25% OOS. Having trouble pulling up stats on IS/OOS on my cell phone but U Illinois has a rural track that puts out just absolutely stellar graduates.
OOS tuition at these schools make them not even worth applying unless OP has a trust fund.
 
Have you looked at Medical College of Wisconsin? To start with, they are a private school so IS V OOS isn’t an issue. They have a couple of campuses In some rural areas. So they might have what you are looking for.
 
Maybe Virginia Tech. It's located in a small/medium size city (Roanoke) but the surrounding area is rural southwestern Virginia. The health system draws from areas all over that part of the state and even from WV. The percent of OOS students is pretty high (more than half).

I was accepted to LMU-DCOM out of state with mediocre stats (but good extracurriculars and clinical experience). They put a huuuuuuge emphasis on rural primary care
 
I’m not sure about their OOS rates but Geisinger has a special scholarship if you commit to rural med for a certain amount of years, so they definitely value it.
