Hi

I am facing the following scenario:
A 10 year old male child presented with cough dyspnea , restlessness, sweating and pain in abdoman for last one day. On examination, his pulse is 110 / min , respiratory rate 5 / min, chest hyper expanded with reduced movement, resonant percussion note , reduced bi lateral air entry and wide spread expiratory ronchi.

What is the diagnosis for the above scenario.

I think that the case scenario could have several possible diagnosis, i.e. a differential diagnosis, some being more likely that others. You should be able to list five or so top things that patient may have. (I don't think the child had a respiratory rate of 5 . . . typo?)

The physical examination findings and the bare bones history could mean a lot of things.

You left out one important pertinent positive (or negative) at least, i.e. does the child have a fever? If so how high? Also, there is no medical history, would it matter if the child had cystic fibrosis?

I would guess:

1. Pneumonia possibly, this can cause abdominal pain and the other symptoms.

2. Asthma possibly

3. Epiglottis?

4. Foreign body obstruction acting in a ball valve mechanism? (Not all that wheezes is asthma)

5. ARDS secondary to aspiration of gastric contents ??

6. Appendicitis? Maybe this is why he is breathing slowly if it hurts when he takes quick breaths??

Well that was a typo. respiratory rate is 45 / min.

1. Since percussion note is resonent so Pneumonia may not be possible.

Expiratory Ronchi and Expiratory Wheez are the same thing or not. If the answer is yes, expiratory wheez is present in cystic fibrosis

Also there is no fever. No Medical history of cystic fibrosis.
