how does one determine which lead to use as the rhythm strip?

Depends on what you're trying to show. For example, if you have an option of which one to show at the bottom of the EKG and you're trying to show a block you pick the one that shows it best, i.e. the one with the biggest/most distinct P wave, in case the P's aren't as visible in the other leads.
