Are they worth the trouble for left sided lung cases, pneumonectomies etc?

For the vast majority of left sided cases, a left DLT works just fine. You can even use them for left pneumonectomies. Either pull the tube back prior to resection if the staple line is going to be high, or leave in place if the surgeon is going to leave a longer stump. One of my attendings loved right sided tubes for nearly everything, though, as he felt they taught better bronch skills.

No not unless there will be resection of left main bronchus...pain to position, easy obstruction, etc. we don't even stock very many.

Bumping.

Doing a left pneumonectomy tomorrow. Any new thoughts? (Two masses, neither are near the left main stem)

Only if you want to be constantly checking the RUL, seriously, I talked an attending into letting me do one once. Never again, spent half the case checking the tube.

Just be careful when the surgeon staples or suture the left main bronchus as to not include your tube. I've heard that happening before.

Edit: didn't read psychbender's old post. Agree with that.
