Does anyone know the address for where we send the supp app materials like the photo and check for the fee? I can't find it on the website.

"Concurrently with submitting the AADSAS application, applicants should send their $75 application processing fee directly to CODM along with a 3x5 index card that provides the individual student’s DENTPIN, AADSAS ID, email, phone and other contact information."
