so heres my dilemma, i just started 4th year and im doing a cardio elective. im not going into cardio or medicine but i figured it would be a great rotation to learn how to better handle cardio patients and their associated pathology. anyways, the rotation has been great- attendings teach a ton, good lectures and case presentations, and a good learning atmosphere all around with good patients to learn from.

my issue is that every day at around 2pm, my resident that i follow tells me to "go home, there is nothing else left for me to do". she thanks me over and over and always tells me how helpful and what a good knowledge base i have. there are no signout rounds specifically for cardio (most of the attendings are at their offices in the afternoon and only come back over to the hospital if there is a serious issue that they are needed for). i feel guilty leaving so early though. my entire 3rd year, i left the hospital at 5 at the earliest. am i just feeling guilty bc of my past experiences or am i doing something wrong? maybe it is just in my head but i was hoping to ask for a letter of recommendation from this rotation and kinda feel like i dont deserve it since im out early each day. any opinions or anyone have any similar experiences?

I wouldn't feel guilty. There are just some rotations that don't require as much from you. If they told me to leave that early, I would ask 'are you sure?' once, then leave. I even got letters from some of these rotations. I even functioned better because I wasn't as tired or sick of the rotation by the end. It's nice when a rotation is efficient enough that you don't have to stay super late. That's the way I see it.

yeah, i always ask are u sure and tell them im more than willing and stay to help to do whatever is needed. i was hoping to ask for a letter from this rotation also. this rotation is helping me prepare for step 2 though (i know how to answer most any cardio question and i have time to do my UW all night).

Correct me if I'm wrong, but if you are a fourth year now, then it is likely pass/fail right? Even if you get a letter from the attending, the residents are telling you to go home and the attendings probably won't see/know/care when you leave. That, and the letter will primarily come from what happens while rounding and unless you do something grossly bad, they'll only be positive. I would say just GO HOME and don't worry!

Also, correct me if I'm wrong any residents, but if you really like a student and think they are working hard, would some of you just send them home early to be nice, kinda as a treat for hard work. I feel like that happened in some of my surgical rotations and I want to go into surgery.

Enough said... relax and enjoy 4th year 👍 I agree that the attending doesn't see/know/care where you at after 2pm.

Some may not care, but if you do this on every rotation you are going to end up with an unhappy attending. Make sure that you only go if the person in charge of your grade tells you to go and that even then you say you are going to read. Have your resident/intern call you if anything happens.

You are so lucky. Enjoy it. 😉 As an intern you won't have this luxury
