I didn't ask if you listed that they were on probation, I asked if you mentioned the problems you had there when you filled out your site evaluation.
Who has put them on probation? For what?

You can inform your school of whatever you wish. If it has been "several years," then the school may already be aware of this, but it certainly won't hurt for you to make sure. I'm still confused about who put them on probation & for what.

If you're concerned about the quality of the site, I see no reason not to mention that to your school.
