Hi,
I will be going on rotations in the fall and have NEVER worked in a hospital. Most sites are VA. I wanted to get advice somewhere along the lines of:

"Things I wish I had known before rotations"--> Can be curriculum related or working with a team dynamics

So that I don't get my butt handed to me too badly when I get there.

My biggest fear is answering a question thinking I know something but being wrong later on after it has possibly harmed a patient. Thanks 🙂.

Some questions:

Were you really expected to understand and know how to use a resp machine and PEEP?

Just how much ID did you use?

How long did it take the team on rounds to actually see that you are useful?

Should you be at the hospital an hour before rounds to review charts?

I hated guidelines because they always changed and felt that sometimes, it was ok to skip a step (Asthma, Bipolar disorder etc) and be more agressive. How important are guidelines? And I don't mean solid ones like JNC-7.

Any other info would be much appreciated. Thanks in advance.

never, unless I actually wanted to make myself useful

first rotation, i was there 2 hours pre rounds. But yeah, you are expected to be there before round to work up pts

pretty important, especially when you go through rotations with faculty. they try to drill it into you

But in all seriousness, its a great time to learn. And people know that you don't know everything. The key is actually taking the time and FINDING THE ANSWER OUT AND GETTING BACK TO THEM. AND NOT JUST A ONE SENTENSE BS. MAKE SURE YOU ACTUALLY THROUGHLY ANSWER THE QUESTION.

At the VA, that place is amazing. My final two rotations were at the VA with anticoag and clinic, i'm telling you, if i had those rotations sooner, i would be a resident coming this month working for them instead of working for cvs.

If you're on a service with lots of turnover, you need to get there very early to pre-round. On a service where patients are in for awhile you can get away with coming in later because you don't need to review the H&P, just labs and vitals.
