Title says it all, was curious, since I think I have a good chance at RU as im instate and have a above RU average DAT and good gpa. Was wondering how hard it is to maintain a high class ranking if you want to specialize. How are your exams and are they more content based or application driven.

Won't sugarcoat it for you -- I'm at Rutgers and this school is no joke. Classes are piled on and really difficult (up to 50 credits in a semester), and sometimes we can have a few weeks in a row with 2-4 exams per week. The content itself is not necessarily super difficult and the exams are more content-based than application based (at least in D1 and D2), but if you can stay on top of your stuff and keep up with the class material, it's totally doable. If you are dead set on specializing I'd explore a med school-integrated curriculum school like UConn or Columbia.
