If you graduate in the top 5, not 5% of your class, but top 5, then are those students more likely to see the highly competitive residencies (subsequent higher starting salaries out of residency)? What is the program that everyone guns for in terms of allowing podiatric surgical residents to participate in the most cutting edge cases?

There are several "top" programs

Dekalb, kaiser, inova just to name a few.

Being in the top 5 would make you eligible to do clerkships at those sites, but by no means does it guarantee a thing. A lot more than gpa goes into picking a resident after they have rotated through a site.

just to name a few: interviewing skills, ability to mesh with the staff, and your abilities

I am sure to other LORs, probably being published would help. A residency it like getting a job, there are many facets that get involved.

I'm not really sure there is a clear cut "Top". It really depends on what you want to do with your DPM training. there are residencies that will expose you more to certain things, like wound care for instance, and less to others( like some don't teach you rear ankle). After talking with a fourth year who has finished all his externships, I feel like it is a lot like rushrhees said, the rotations are like a monthlong interview. They get a good feel for you and if you are a good fit for them. You also evaluate them as well. So while GRADES ARE IMPORTANT, they are not the be all end all.
