how possible is it to scramble for a transitional year spot? Or do basically all of those programs fill via the match? I am applying for PM&R programs, and feel good about my chances for matching for PM&R. I am concerned though about matching for the transitional year, and I am wondering what can happen if I don't match for that year.

There are some transitional spots left after the match but they are likely grabbed up quickly. One must wonder why some places dont match.According to Scutwork.com Maryland General Hosp has had many unfilled transitionals for the past few years. There are always the prelim medicine spots which should be available someplace if really needed. Its best to avoid the scramble if at all possible.
