Hi, I am a Peds resident, planning on finishing my 3 years in this residency, debating doing a second residency in EM.
My questions are:

1. Anyone out there who has done this recently?

2. Anyone know how funding for this will work? I understand I will have used up 3 years of funding to pay for my 3 years in peds. Would I get 2 more years of funding to be an EM resident?

3. Does anyone know how much credit you get from a Peds residency that can be taken of off a 3 year or a 4 year EM residency?

4. Anyone aware of programs that would be open to accepting an already peds resid grad?

If you don't have answers but can refer me to sites or people who would have answers, I would appreciate that as well.

Well first off it's not clear if this is the best route for you. What do you ultimately want to do? If it's work in a peds ER, then you may as well just do peds EM fellowship. But let's say you do want to deal with adult medicine, it would depend on the location but I can envision yo getting credit for at least PICU time and maybe credit for your peds ER block (i think those two are pretty standard across most programs).
 
I know someone who did this - he works full-time for a busy community ED and does a few shifts/mo at a level one children's hospital ED - did not do a peds EM fellowship. I don't know about the GME funding details, just that this guy did it...
 
HI pf.....while it may not answer ALL your questions, here are my $0.02

- Dont know about peds to EM, but do know about family med to EM
- Funding: lots of threads on this issue, with lots of confusing details. You can look at the actual guidelines on the pubs page of the AAMC (i have the actual one, cant figure out how to post an attachment here).

In a nutshell - and tailored to your case: You've used up all your funding for peds, correct. ANYthing you do now, including fellowship, or another residency, will be funded at 50% direct (salary) + 100% indirect (fringe+benefits) from Medicare. That leaves the hospital to cover the rest. As you can see, its not 0, but cash strapped hosps. may have a problem, others wont. Can be used as an excuse not to take you, but if some place really likes you, and you're stellar (in their eyes) it's not a huge deal. Many others have switched in the past.

- Lots of threads on switching, search for posts from aProgDirector.
Dont know about your other 2 questions - but the websites that contain open positions are generally findaresident.com and residentswap.org and openpositions.com
Then there are specialty specific ones like the surgery prog directors site, AMA for IM, AAFP for FM, I dont know about peds or EM...but am sure there are...

1. I know at least three persons who did this, and now they are EM attendings at a tertiary care hospital.

2. Funding has to come from hospital now, might be an issue for some places as others pointed out as well, but not impossible.

3. I doubt if you'd get any credits in the new residency, I know of IM residents getting about 6 months off, but thats coz there is some overlap in training of EM and IM (e.g MICU and floor months), but I doubt if this would be the case with peds resident. At least the persons I know had to do entire 3 yrs.

4. Your best chance would be at the hospital you trained at for peds, if they have an EM residency, it would be your best bet. If not, you might have to apply far and wide.

One of my partners did this and attends in our tertiary care ED and the peds ED. We're not the major pediatric hospital in the city, so our peds ED docs admit to themselves for simple stuff and transfer the rest.
