What if it's in a niche area that you're known to excel at?If a patient sees another primary care physician without discussing it with me first, that's a transfer of care, and they can't come back.
I won't do second opinions for other PCPs, either. If the patient wants to transfer care to me, fine. If not, go elsewhere.

If someone wants a PCP who does something I don't do, it's just easier to transfer as opposed to seeing us both.The only exceptions I've made over the years were for a handful of patients who wanted to see somebody in our group who did bariatrics or OMM. Since we all (me, the patient, and the other physician) discussed it beforehand, I allowed it. It still ended up being awkward, however.

If NDs aren't an issue in your state, prepare for it.
In my parts they are dosing out zyprexa, transgender hormones, mood stabilizers, etc
