depends...a few of mine just asked for money....some ask questions like "why our school" or ask you to elaborate on your activities or stuff like that. some ask you to attach a 2x2 passport size photo so if they interview you, they can remember you better when they're reviewing your file.

It varies considerably from one school to another. Sometimes they just want a check to "reaffirm your interest", some ask for you to elaborate on your future plans as a doctor (ie, What are you goals and describe what you have done to begin accomplishing them), some may ask for additional experiences or circumstances not mentioned previously that you would like them to consider, why do you want to attend med school X, etc. The point is that most will ask for additional essays but what they are depends on the school. I found that they were often much more work than the primary application.
