Secondary app prompts

So I have begun receiving secondaries and I'm confused about some questions. Some schools ask for a "describe a challenge" essay and others ask for a "describe a challenge/obstacle in applying to medical school." What's the difference? Do I take the latter literally? Not exactly sure what they're looking for with the second prompt...

Also, the second prompt has 200 word limits and the first has 550 word limits

I would recommend taking the questions as literally as you can. Don't want to get dinged on a technicality! I think something you can use for the second, you can use for the first, but not necessarily the other way around.
