Does anybody know the situation with the following secondaries? How do you access them (i.e. through the web or send in an AMCAS paper app with a request)? If it is through the web, what is the address? Thanks.

OHSU (I've heard they are waiting for the full app)

Boston has no secondary. They will send you a request for money. MCPH and OHSU are waiting for AMCAS to get their act togehter.
I don't know about the others. Have you checked out Drako's website that tells which schools want paper apps and which schools have their secondaries on-line? It's at: http://www.geocities.com/lvnh6791/paperamcas2002.htm
