I'm starting my residency at a surgical residency that many people would consider big name and competitive. The surgical interns, including neurosurg, ortho, ENT, plastics, categorical, literally come from all over the spectrum of US medical schools. Some went to really top name programs, others went to programs that many on this board would call "lower-tier" or "less-competitive" US med schools (Ros Franklin, etc.)

We're all interns. Be at a place that makes you happy enough that you can be successful as possible. If you work really hard and do what you're told. Residency options are those that you create for yourself through your effort and success in medical school.
