Recently I had been to a Physical Therapist was talking about My Medical school application. I asked him if I could shadow him for a week. He agreed. Is it a good idea to use shadowing hours with Physical therapist for Medical school application or Do we need to do it with Physician only.

I know they dont prescribe medications, but do understand physical and Biological nature of the human body and they study and deal with it from different perspective.

It is not a substitute for shadowing a physician but it can be a good way to enhance your idea of "why medicine" and "if not medicine, what would you choose to do?" which are questions you might be asked at an interview.

As we need to do shadowing with multiple specialities Physical therapist will be one of them. Also thinking of shadowing with a General physician and a opthalmologist.

You don't "need" to shadow multiple specialties beyond someone practicing primary care (which you mentioned), but it can be fun to get other points of view of medical practice.

Possibly the physical therapist can refer you to an orthopedic surgeon or sport med doc who is willing to be shadowed.

Shadowing different jobs might hurt your application more than a neutral effect. It definitely won’t improve your application. Schools might wonder if you really are all in on becoming a doctor as opposed to something or being unsure. Now if someone shadowed a PT before becoming premed it might not come across that way. But still wouldn’t help your application.

You can shadow anyone you want. You don't have to put it on your application if you are afraid that it might "hurt" your application to show an interest in the roles of other medical team members.

Well sure and never said you couldn’t. Just doesn’t make since to put it on your application.
