Shadowing and Volunteering

I have been reading the forums for awhile now, but I haven't really seen this question addressed. When filling out your application, what documentation (if any) is needed to verify your shadowing and volunteering hours? Or is it basically an honor system?

I will be applying next cycle and I was just wondering.

Good to know since my mentee and the program coordinator went cold turkey on me lol. I'll still put down the 20ish hours that I did get
