It looks like the application has a spot for dentistry experience and a short text area to give more details. Should I write this description for each dentist or all of the dentists as one shadowing experience?

If each of the dentists were from different practices than I would list them separately. And the short text area is very short! I put something to the effect of this in those areas:
I assisted restorative, oral surgery, and endodontic procedures. I took traditional and digital xrays. I took impressions, placed sealants, and fabricated temporary crowns.
