So I shadowed about 4 years ago freshman year..

Do we need to know the contact person for that? I dont remember because it was at one of those large mills and I basically just floated around for 2 days..

If your using them for a LOR then contact them, but if you just shadowed the little bit you did awhile ago, just enter the contact information on your AADSAAS (Name, address, your title, hours, dates, etc.) .
 
Realistically, if that is all you have shadowed, I would shadow a lot more than that. Two days is nothing especially when it was that long ago. For some schools, that doesn't even come close to meeting their minimum required hours. But if that is all that you have done and you are applying, do the best you can do to get the information on the dentist. Contact that dental office and see if they know who the dentists were who worked there four years ago. Explain to them that you are applying to d-school and need to see if they can give you his/her contact for verification of hours shadowed if needed by the schools. If you cannot get his/her contact info, then all you really can do is list the dental office's office name and contact info. If questioned during the interview, explain to them the situation and they should understand unless they need a reference letter from the dentist like Baylor does.
