Shadowing Record Keeping?

Is there a certain way you should record your physican shadowing experiences??? Do you need the doctors to sign some sort of paper saying you shadowed them?

I am a sophmore so I have time but I am getting all the wheels turning to a complete well rounded application!

No records are kept. Most of the med school application process ends up being on the honor system. There is a place to list contacts, but no one really checks them -- especially for something as trivial as shadowing.
 
My pre-med committee requested I list the number of hours I shadowed with each doc. They use that information in their composite/cover letter. Plus, as has already been mentioned, it's sometimes nice to remind the doc how many hours you spent with them if they're going to write you an LOR.
