I was consulted to see a depressed patient on the floor this morning. Depressed due to medical problems. Had a trial of 3-4 antidepressants in the past but had side effects. However last summer he accidently electrocuted himself. After that he had improvement in his mood, increased productivity and perceptual abilities for 3 months. Now he wants ECT!😀

Some days are so fun.

All I hear is you telling me to stick a fork in a socket when residency gets tough and things will get better. 🙂

Sorry won't work. You have to get the circuit to cross at least part of the brain.

I recommend "surprise unilateral ECT"...
hotwire a standard phone handset and wait for it to ring.

I heard that Mossad was pretty good at that - well, more with "construction" (plastique) than electricity.

You ought to publish this case. Get this guy's permission, and submit it to a journal.

Seriously.

It's just heresay at this point. However, since he has lost hs job and house and only has Medicaid maybe he could repeat his original shocking incident with witnesses.

Most likely wouldn't be medically cleared for ECT either way given his multiple medical problems. Also, you would have to determine if his depression was the result of the medical conditions being a direct physiological consequence of his general medical condition (e.g. Parkinson's, heart disease, etc), or a comorbid major depressive disorder. The literature suggests that ECT wouldn't be beneficial if his depression is a consequence of the former (as is the case with substance abuse/dependence). Agree with Kugel, the fact that he was accidentally electrocuted did not produce a controlled seizure--and is therefore of no use.
