From what I've gathered here, only a few schools require proof and I believe that ask for that in the secondary. While doing my shadowing I would always just mention to the dentist that I may have to get proof one day in the future and they usually didn't really care so I think you should be ok!

I know we don't have to prove anything to report them on aadsas but if we had to prove them to a school how do they usually want us to do it? I have my hours but I was told we didn't need to get anything filled out so I never did. And what schools require this for the supplemental application?

Again, most dental schools suggest you submit a letter as one of your four evaluations from a dentist you've shadowed which is proof enough, the dentist can list the hours you've done. Tufts is notable for proof of shadowing requirement.
