Without an MCAT it's nearly impossible to determine. If you maintain your academic performance and score a fairly good MCAT score, I'd say you're pretty competitive for MD-PhD programs. Even without an engineering background, you can still have enough experience from working in an engineering lab that you can enter an engineering PhD program and gain the academic experience necessary (be prepared to justify why you didn't pursue engineering in undergrad, though).

I can't comment on the rest of the post (regarding recommendations).
