An article that I had worked on last year (clinical research) was accepted for publication this last week and is in press. I am the first author. I had mentioned in my AMCAS description that I had worked on a manuscript and that it was in review with a journal, but I did not mention that I was the first author. Does it make sense to write a small update now if a school offers the opportunity to do so after submitting my secondary? If I should write an update, is the proper form something that looks like a letter? Who should the update be addressed to (admission committee)?

I think the process for submitting updates varies between schools. Some schools accept them, some schools don't, some schools accept them but don't read them. At schools that do take it, it's usually just uploading a file of some sort to one of their portals addressed to the admissions committee as a letter.

2) No. Don't overthink this. Just a note stating "the article listed as submitted is now in press at ...."
3) Just send it to the Admissions Dean.

No, not an update. You should say this on your secondaries. Sending an update before secondaries has a poor optics

Not every school has a place on the secondary to write something like "is there anything else you would like to let us know". But some schools have the option to send "additional documents", would that be okay?
