So my program just got this email for a trial run in order to make the sign-outs more efficient.
Is there anyone out there already using this?
If so, any thoughts? 👍 or 👎.
If possible please let me know what the bad things are about this if you do not like it.

Part of the problem with the Word/Excel is non-HIPPA compliance. Unless everybody uses just one computer, you have to save it to disks or email it to yourself in order to be able to use it freely.

We have PowerChart so we can get the pre-printed notes with the labs already in it, so that aspect is not a big draw, but the ability to use it from home to update labs etc is definitely a big plus.

How do you guys use your lists specially with the new duty hour rules with the overnight intern not necessarily being able to get first hand sign-out from interns that have left earlier in the day.

Secure server at your institution. It's basically free and can be controlled by your institution.

We have a PW protected server available only to those who need access where sign-outs are kept, usually in Word or Excel format but that is up to the person that makes them. The security settings are complicated but well done. I can see the sign-outs for my sub-specialty but not the general IM services or other IM subs. I can't see any other signouts (Surgery, FM, Gyn, etc) and vice versa.
 
We have an Access database that was configured by our previous program director to be very user friendly. It is on a secure drive so only accessible within the hospital and only to those who know it exists. Can be accessed on any computer that has the program (all the call rooms, computers in the department office, the ICU break room computer) so no bottle necks. It gets a little confused if two people are trying to update the same patient at the same time, but otherwise can be updated by multiple people simultaneously. The night float is responsible for updating the list (adding new patients, changing room numbers, new issues) overnight. As for actual signout, this is done in the morning by the night float to the teams (there are only three services since we are a small program-2 general teams and trauma) with whatever the intern needs to add about calls they received. In the evening the night float is the same sunday through thurs night so the chiefs may just call to update the night float (they sign out, not the interns although if a third or fourth year is on service they get tasked as jr chief and will take care of signing out). Trauma has to be in house until the night float gets there so they sign out in person. The on call intern is generally there, but may be working on something and will get the info from the night float later. I don't know how this would work for a bigger program.
