Question 32 of 100 from the 1998 Micro released test...

Each of the following is commonly associated with congestive heart failure EXCEPT one. Which one is this EXCEPTION?

Dyspnea
Cyanosis
Anasarca
Ankle edema
Passive congestion of the liver

The answer is listed as Anasarca but I am unable to find anything giving me reason to think it is not commonly associated with CHF. The only reasoning I can come up with is that it is caused by other organs and seen in CHF.

anasarca is a generalized edema in which the fluid collects in extracellular spaces and is mainly d/t low plasma proteins or defect in lymphatics to clear up the interstitial fluid but not seen in congestive heart failure(where edema occurs primarily due to back flow of blood thru the veins as the heart is not working)

I would have to say there are no exceptions. Anasarca is massive edema, not dependent on origin. However I was taught that sometimes all could be correct answers and to pick out the best. In that case I would go will anasarca b/c it would be the one of the last clinical signs of CHF. However it will occur due to the increased intravenous pressure leading to generalized edema.

I actually looked at the exams that I have and the person that I got it from has an X through the question and has written "no exceptions" next to it.

But like I said if I had to choose one it would be anasarca since it would occur last in those given symptoms.

Sweet, I am glad yours is keyed as no exceptions- at least I am not losing my mind. I agree with everything you said- thanks for the input!

Ankle Edema, Pass congestion of liver and raised JVP(not mentioned here) are classical signs of right sided heart failure....Dyspnea and cyanosis are also common signs of heart failure....I just know this from memory...
