Concerning volunteer/shadowing/clinical volunteering hours.

Do I have to have them logged somehow? If so how? I've seen sites where one can log them.

I've actually looked up info on this and nada.
 
If I understood your question correctly, no you don't have to. Some hospitals or other organizations want you to count
hours, so your supervisor or computer system might show you the exact number of hours. However, for the purpose of filling out AMCAS primary application, usually people provide the best estimate for their hours.

Ah- so it's just like a job then. They'll probably take your word for it, but one should still provide information to validate their claim.

What if it's done through a service which no longer exists? For example as a child I worked for a company that no longer exists. There are ripples in time where there is evidence of their existence, but there'd be no one you vouch for me if, for some odd reason, an employer wanted to contact them

Typically, you do not include activities in your medical school application that happened before you went to college. This includes high school basically none of your "end dates" should be before you started college. There are a few rare cases - say you went to the Olympics, got a publication, got an award for running into a burning building...etc

Gotcha'. That was just an example. More up to date- say I volunteer with the band of merry mutants during my college years . They get destroyed by the brotherhood of evil. I still volunteered for them, but there'd be no one to say "yeah they volunteered here." What should happen in a situation like that?

Find someone to send you back in time and warn them of their impending doom to save them.
