How can one make a transition and what sacrifices and challenges, from a hospital that is only adults, infectious disease, General surgery, behavioral unit, icu, etc. to a midsized 300 bed facility with cardiovascular surgery, chemotherapy, nicu, labor delivery, adjustment tpn and peds, mostly staffing but some clinical via protocol?
Is it advised?

Pretty vague question. What is your role going to be? Are you going to be just in the pharmacy verifying orders? Are you going to be in specific areas performing rounds? You said staffing, so I would assume simply being in the pharmacy verifying orders. That should not be a major transition issue, as you will just need to know additional resources to use to help with areas like Oncology and NICU. Typically institutions that have these specific areas have a lot of policies and procedures in place to limit variances that could lead to errors. Now if you are going to be sent out to specific areas to be the pharmacist over that area for the day, you could find yourself in a sticky situation if you are unfamiliar with typical questions/scenarios you may encounter.
Additionally, I would say it is never a bad idea to gain additional clinical experience, as that experience will be valuable if you ever had to look for a new position.
